Position Summary:

The Senior Director, Patient Navigation will play a crucial role in leading ZERO Prostate Cancer's innovative patient navigation program aimed at improving outcomes and reducing disparities in prostate cancer care. This position will oversee the implementation and management of four distinct patient navigation pilot models, ensuring their success and impact across various healthcare settings and communities.

 

Key Duties & Responsibilities: 

1. Strategic Leadership:

  • Develop and execute the overall strategy for ZERO's patient navigation program
  • Collaborate with the VP, Community Engagement and Outreach and the Measurement, Evaluation, and Analysis team to align navigation initiatives with organizational goals
  • Participate in the national “Blitz The Barriers” Partnership Committee to drive engagement and decision-making around project initiatives
 

2. Program Management:

  • Oversee the implementation of four patient navigation pilot models: 
    • Practice-Based, Community-Based, Volunteer- Based, and Centralized/ Virtual
  • Establish protocols and best practices for each navigation model
  • Ensure HIPAA compliance and data security across all navigation services
 

3. Team Leadership:

  • Hire, train, and manage a team of patient navigators and support staff
  • Provide ongoing professional development and mentorship to navigation team members
  • Foster a culture of excellence, empathy, and continuous improvement within the navigation program
 

4. Stakeholder Engagement:

  • Build and maintain relationships with healthcare providers, community partners, and other stakeholders
  • Collaborate with the Community Engagement Committee (CEC) to tailor navigation services to local needs
  • Represent ZERO in relevant industry conferences, meetings, and events
 

5. Quality Assurance and Evaluation:

  • Work closely with the Director, Measurement and Evaluation to develop and implement performance metrics for the navigation program
  • Analyze program data to identify trends, challenges, and opportunities for improvement
  • Contribute to the development of white papers and dissemination of findings
 

6. Resource Development:

  • Collaborate with the Manager, Mission Volunteers and Engagement to integrate volunteer support into navigation services
  • Develop and maintain a comprehensive database of national and local resources for patient referrals
  • Create educational materials and training resources for navigators and patients
 

7. Innovation and Continuous Improvement:

  • Stay informed about best practices and emerging trends in patient navigation and prostate cancer care
  • Propose and implement innovative solutions to enhance the effectiveness of navigation services
  • Adapt navigation models based on pilot outcomes and stakeholder feedback



 

Qualifications:

  • Bachelor’s degree in Public Health, Healthcare Administration, Registered Nursing, or related field
  • Minimum of 8-10 years of experience in patient navigation, oncology care, or related healthcare field
  • Proven track record of developing and managing successful healthcare programs
  • Strong understanding of prostate cancer care, treatment options, and related challenges
  • Excellent leadership and team management skills
  • Outstanding communication and interpersonal abilities
  • Experience working with diverse communities and addressing health disparities
  • Proficiency in data analysis and program evaluation
  • Knowledge of HIPAA regulations and healthcare compliance requirements
 

Preferred Qualifications:

  • Compassionate and mission-driven leader with a commitment to improving prostate cancer care and reducing health disparities.
  • Proven ability to navigate complex healthcare systems effectively.
  • Strong track record of building and maintaining strategic partnerships.
  • Inspirational leadership skills with the ability to motivate and guide teams toward achieving exceptional results.
  • Experience in healthcare, advocacy, or nonprofit sectors is a plus.
 

Physical Requirements:

  • Constant use of a computer and other office productivity equipment, such as computer printer, calculator, and copier. 
  • Prolonged periods of a stationary position at least 50% of the time.
  • The ability to recognize details at close range for extended periods of time.
  • The ability to communicate information and ideas so others will understand. Must be able to exchange accurate information in these situations.

This role will report directly to the Chief Mission Officer and will work closely with the Mission Team to support ZERO’s mission to end prostate cancer. 

 

This position is based remotely and candidates may live in any area within the United States. This position will require nationwide travel approximately 20% of the time in a year
